Your smile says it all! We feel that everyone deserves to have a
smile they can be proud of. That is why we are excited to offer our
patients a unique program we call "Brighter Smile for
Life." The program is simple - as long as you stay current
with your regularly scheduled dental exam
visits, we will give you whitening gel absolutely
free for life!
There are many reasons teeth can appear dull or unhealthy. The
first step to restoring beautiful tooth color is to schedule a smile
evaluation to determine why your teeth are not their
whitest. Tooth staining from food, drinks, smoking, etc., may
require a long-term care program to restore and maintain their
brilliance.
When you come to our office for your preventive examination,
x-rays and cleaning, we will provide you with custom bleaching
trays and materials for a one-time enrollment fee. Then, at each
recommended preventive visit, we will give you a complimentary
touch up kit of bleaching gel.
About The
Program
With our "Brighter Smile for Life" program you
receive:
- One set of customized upper and lower trays with starter kit
for a one-time fee of $99*.
- A touch up kit of whitening gel at each regularly scheduled
exam at no charge.
To be eligible for the program, you must meet the following
criteria:
- Be a new or existing patient of record.
- Have a complete dental examination by us with recommended
x-rays.
- Do not have active, untreated disease, gum disease, decay
(cavities), root sensitivity, oral cancer, etc.
- Agree to and sign whitening consent form.
- Stay current in keeping with your recommended cleaning
appointments (Prophylaxis, periodontal maintenance.)
- Your account must be current (No outstanding bills or insurance
claims.)
* $99 fee due at time of tray impressions.
Once enrolled, as long as you keep your regularly scheduled
dental exams, you'll receive additional whitening gel each visit
for the rest of your life!
